Health Outreach Partners acknowledges and celebrates Asian Pacific American Heritage Month. We commemorate key moments in Asian Pacific Islander American history. On May 7, 1843, the first Japanese immigrants arrived in the US, while May 10, 1869, marked the completion of the transcontinental railroad, largely built by Chinese immigrants. In the 1960s, Larry Itliong organized thousands of agricultural workers and was one of the founders of the United Farm Workers. Let’s honor these milestones and their contributions in shaping our nation. Join Health Outreach Partners and Community Health Center, Inc. for a free two-part webinar series sponsored by the National Health Center Training and Technical Assistance Partners. This series is designed to provide participants with foundational knowledge on utilizing innovation and team-based care to address maternal health disparities during and after pregnancy. In Webinar 1, expert panelists will discuss an advanced model of team-based care that can be used for addressing maternal health disparities. The panelists will dive into the application of team-based care strategies and the roles within the interdisciplinary team including the primary care provider, certified nurse midwife, registered nurse, behavioral health, and more. The second webinar will be on the topic of maternal mental health. Expert panelists from Alameda County in California will discuss innovative programs that incorporate mental health for new and expecting mothers, as well as hear about challenges and successes when implementing mental health screening tools in health centers.
